The other two tabs include the forms for submitting the 2012-13 Perkins IB Consortia Project Grants final budget information.

The information below has been copied from the original RFA that was sent out last year. Please note two (2) changes that were made last year. First, we need only two (2) signed copies of the final report. Do not send the report in a hard cover such as a 3-ring binder as these reports must fit in a folder in our file cabinets.

Work Products

  • The grantee will provide a list of consortia committee members, their titles, and the employment sector, local educational agency, or regional consortium they represent; one set of advisory committee meeting minutes; and a copy of any significant printed material produced with grant funds; in addition to the final report questions in the original RFA. 
  • All products that have as a final creation any media software (e.g. DVD/CD) Send ONLY one (1) copy of each of these products to CTE, in electronic format.

Sign the Final Report of Expenditures by Source of Funds and the Data Summary forms and submit to the Chancellor’s Office as directed. Three copies of the final report must be submitted; all two (2) of the copies must have original signatures (all signatures must be an ink color other than black).

Final Report Information

Work Products 
The grantee will provide a list of collaborative advisory committee members, their titles, and the employment sector, local educational agency, or regional consortium they represent. Also include one set of steering committee meeting minutes; and a copy of any significant printed material produced with grant funds; these additional printed materials should be sent in electronic format.
Do not send the report in a hard cover format such as a 3-ring binder as these reports must fit in a folder in our file cabinets.

Please note that while the majority of these documents are the same as in previous years, there have been a number of changes. First, a number of grants were listed separately and the same information was repeated for each, this duplication has been removed and the RFA has in turn been reduced by 8 pages. Second, we are now asking for four (4) signed copies of the grant application; you now sign all four copies instead of just three of four. Third, we need only three (3) copies of the final report and they all must be signed. Fourth, we want only one (1) copy of any products, such as CD’s, notebooks or brochures that out you produce.
The most significant change is to the requested Final Report Narrative of outcomes. The number of questions has been reduced to 7; some questions have been deleted, new ones added and a few combined. We hope this leads to a more clear-cut process and responses that will provide this office with data that is of added relevance to our analysis of outcomes.
